Preschool Special Education Form

Parents, please complete this form for your child if you would like them evaluated for the Houston County Special Education Preschool program. Please complete as much of the information as possible. This will assist us during the referral process.

Child Find

Child Find is a statement effort by the Alabama State Department of Education and the Department of Rehabilitation Services to locate, identify, and evaluate children with disabilities from birth to age 21.
